Sport Physical Examinations
Physical examinations for the 2009-10 school year will be on the following dates. These are the only times free physical examinations will be given. You must choose to have Dr Autotte to be your health care provider for your child. All physical examinations forms must be done on the High Point H.S. athletic form. If you choose to have your own personal physician to do the physical, you must check off on the High Point Physical Exam form that you agree to have Dr. Autotte review the medical records of my child from my private physician.
All forms may be picked up in the main office at the high school.
2008-2009 Physical Dates To Be Announced
They will be in the nurses office at High Point Regional H.S. and the times are from 8:15am to 3PM. Sign ups will stop @1:30 so that they may stop at 3PM. Please call Mr. Parise @ 973-875-0020 with any questions.

Self-Administration of Medication
5330 Self-administration of medication
for asthma or severe allergic reaction
* As of December, 1993, State law permits pupils to self-administer medication for asthma or other potentially life-threatening illnesses in school.
* Written authorization
for this is required from the physician and from the parent/guardian annually.
* The attached forms are provided for this purpose. Please return completed
forms for the current school year to the school nurse.
